Student Application We look forward to giving you the best training we can in a safe envirorment. We value your safety and want you to have an enjoyable experience. With that said, we ask that each of our students complete and return a student application and attendance at any class is subject to approval of this student application. The information provided in this form will only be used to complete a background check so that you can feel safe that the person next to you is also law abiding. Some classes are restricted to active duty law enforcement personnel while others require previous training experience. FAST reserves the right to reject students not meeting these or any other qualifications we may require. Any student that does not meet the class requirements will have funds returned. Please e-mail or call if you have any questions. Please complete the student application and fax, mail, or email it to us.
You MUST complete and submit a Student Application 4 days prior to the start of your class to be accepted for training. (only once in a 12 month period)
Payment You can use the shopping cart to register for classes online, or call in payment information. We accept Visa, Master Card, and Discover. We can take your credit card over the phone. No one will be confirmed in a class until we have received full payment. We will be glad to put your name on the class list, but you will not be guaranteed the slot until we receive payment. If the class fills up, you will be dropped from the class list if payment has not been received.

Due to Insurance restrictions no pets, children (unless they are 12 or older and attending training with a parent), or guests are allowed in the classroom or on the range during training.
I understand that if a class is cancelled, my payment will be fully refundable. If I cancel more than 30 days prior to the first day of the class, my payment is totally NON-REFUNDABLE, however the full amount may be applied to hold a space in another available class. If I cancel 30 days or less before the first day of the class, my payment is totally NON-REFUNDABLE, however 1/2 my payment may be applied to hold a space in another available class.
